Treatment and Administration of Medication Form
To ensure our school nursing staff can provide proper medical care, manage emergency situations, and administer necessary medications during the school day, all students must have a completed and signed Student Health Information, Consent for Treatment, and Medication Administration Form on file for each school year.
If you have not completed a form for the current school year, please print and complete the form then return it to your child's school nurse.
If you have any questions regarding student health services or medication administration policies, please contact your child's school nurse.
